症例を英語で説明してみよう──SOAP・ICF・目標設定の英語表現【英語×OT 第8回】

はじめに──臨床を英語で語る力

前回の記事では、メール・SNS・自己紹介の書き方を学びました。本記事では、さらに一歩進んで臨床の内容を英語で説明する力を身につけます。

症例を英語で説明できると、以下の場面で力を発揮します。

• 国際学会でのポスター発表や口頭発表
• 海外OTとのケースカンファレンス
• 英語での症例報告論文の執筆
• 留学や海外研修での臨床実習

すべてを完璧に書ける必要はありません。定型パターンを知っていれば、臨床情報の70〜80%は英語で表現できます。

SOAP形式の英語版

SOAPの各要素

SOAPは日本語でも英語でも同じ構造です。

Subjective（主観的情報）

利用者本人が語った内容を記録します。

頻出表現：

• The client reported that... （利用者は〜と述べた）
• The client stated, "..." （利用者は「〜」と述べた）
• The client expressed concern about... （利用者は〜について心配を示した）
• The client denied any... （利用者は〜を否定した）
• The client reported improved... since last session. （前回以降、〜が改善したと報告した）

記録例：

S: The client reported difficulty with buttoning his shirt. He stated, "My right hand feels stiff in the morning." He expressed a desire to dress independently before discharge.

Objective（客観的情報）

評価結果や観察内容を記録します。数値や具体的な動作を客観的に記述します。

頻出表現：

• ROM: Right shoulder flexion 120°（右肩屈曲120度）
• MMT: Right grip strength 15 kg（右握力15kg）
• FIM score: [項目] = [スコア]
• The client was able to... with [level of assistance]（〜を[介助レベル]で遂行できた）

Assessment（評価・分析）

客観的データに基づくOTとしての臨床判断を記述します。

頻出表現：

A: The client is making steady progress in upper extremity function. Improvement in ROM and grip strength has translated to functional gains in dressing. Plan（計画）

今後の介入計画を記述します。

ICFの枠組みで記述する

ICFの英語表現

ICFは国際共通の枠組みであり、英語での症例記述に最適です。

ICFを使った症例記述例

Body Functions: Decreased right upper extremity ROM (shoulder flexion 130°), reduced grip strength (18 kg), impaired fine motor coordination, intact cognition (MMSE 28/30).

Activities: Modified independent in basic ADL except dressing (minimal to moderate assistance required). IADL: requires assistance for cooking and housekeeping.

Participation: Currently unable to participate in previous leisure activities (calligraphy, gardening). Expressed desire to return to community calligraphy class.

Environmental Factors: Supportive spouse at home (facilitator). Two-story house with bedroom on second floor (barrier). Public transportation available within walking distance (facilitator).

Personal Factors: 72-year-old male, retired teacher, values independence and intellectual activities.

目標設定の英語表現

目標の書き方：SMARTの原則

英語圏では目標設定にSMART原則がよく使われます。

目標の定型パターン

短期目標（Short-term goal / STG）：

[Client] will [動詞] [活動] with [介助レベル] within [期間].

例：

The client will don an upper body garment with modified independence within 2 weeks.

The client will complete a simple meal preparation task with supervision within 3 weeks.

長期目標（Long-term goal / LTG）：

[Client] will [動詞] [活動] independently [条件/場面] by [時期].

例：

The client will perform all basic ADL independently by discharge (estimated 6 weeks).

The client will return to community calligraphy class with modified techniques by 3 months post-discharge.

演習：症例シナリオ

以下の症例を英語でSOAP記録にしてみましょう。

症例情報（日本語）

68歳女性。右片麻痺（脳梗塞後2週間）。利き手は右手。元主婦で、料理が趣味。「早く自分で料理がしたい」と話す。右手のBrunnstrom Stage III。FIM食事5点（監視）。座位バランスは概ね安定。

模範解答例

S: The client expressed a strong desire to cook independently. She stated, "I want to prepare meals for my family again."

O: Right hemiplegia (2 weeks post cerebral infarction). Dominant hand: right. Right upper extremity: Brunnstrom Stage III. FIM eating: 5/7 (supervision). Sitting balance: fair to good with occasional loss of balance during reaching. The client was able to use adapted utensils (built-up handle spoon) for self-feeding with supervision.

A: The client shows motivation and fair physical potential for ADL improvement. Current limitations in right hand function impact self-feeding and IADL (cooking). Sitting balance is adequate for tabletop activities. The client would benefit from graded upper extremity activities and adapted cooking tasks.

P: STG: The client will complete self-feeding with modified independence using adaptive equipment within 2 weeks. LTG: The client will participate in a simple cooking task (e.g., making rice balls) with minimal assistance within 6 weeks. Initiate right upper extremity functional training. Trial one-handed cooking techniques. Educate the client on adapted kitchen tools.

まとめ

• SOAP形式は日英共通の構造であり、英語の定型表現を知れば今の臨床記録を英語に変換できます
• 介助レベルの英語表現（independent → dependent の7段階）は必須の語彙です
• ICFの枠組みは国際共通言語として、英語での症例記述に最適です
• 目標設定はSMART原則と定型パターンで、どんな症例でも英語で書けます
• まずは自分の担当症例を1例、英語でSOAP記録にしてみましょう
